We ask that you print and complete each of the following forms prior to your initital appointment with us. Please bring them with you to your first appointment as they are necessary for the electrologist to go over with you before treatment. If you cannot complete them before your visit with us, we are happy to provide them for you at the office. In this case, please arrive 15 minutes before your appointment time to fill them out.
